		<description>A resounding YES! to Andrew. Traveling the Commonwealth and the nation, talking with patients, families, and consumers, one message is very clear: healthcare delivery is just piece work.  For most patients, no one is responsible to connect the pieces but the patient and those who care for them.  They are tired and scared of being their own medical home (they need help--a partnership) and they want a system of care.  To achieve it requires that we align the structures and processes to support this system of care.  That includes payment and financing.  Fee for service will never give us anything but piecemail.  Will it be hard--absolutely. As the QCC has said, we have to recognize their could be winners and loosers and we have to be respectful of both.  In our current healthcare delivery design, too often the patient and those who care for them can be the loosers.   Healthcare reform, and the healthcare community, has the power to do so much better than that and show others the way.
