		<description>I believe physicians who use home health care services do so because they’ve experienced positive results with hundreds of patients. Home health care services offer them a way to reinforce their instructions, reduce calls to their office, react efficiently to changes in the patient’s condition, avoid emergency room visits, and support family caregivers.

There is much pressure on physicians and home health care providers related to health reform as our society ages and the need to service more patients with fewer resources. Collaboration between the physicians and home health care will be crucial for maintaining leading practices of care and quality patient outcomes in these changing times.</description>

		<description>I agree with the conclusion that building on the relationship between primary care physicians and home care providers can improve the quality of post hospital care and much more.

Quality home care can reduce the number of patients being readmitted to hospitals. It can minimize stress on care givers and in the longer run, more and more individuals are choosing to be cared for in the home rather than choose some other form of long term nursing care such as nursing homes or even assisted living facilities.</description>
